Student Forms

  • Add/Drop Request

    The change of schedule request must be submitted to the Registrar before it can become effective. All changes are subject to the Vice President of Academic Affairs and office of the Registrar. 

  • Change of Degree Request

    The new degree plan is filed with the Registrar, and transcripts are re-evaluated. Changes in plans may be approved only by the Vice President of Academic Affairs. 

  • Change of Records Request

    To maintain accurate records, students can submit this request if their name, address, social security number, or emergency contact has changed. 

  • Concurrent Enrollment Request

    Request to take courses at institutions other than ABU. Approvals for concurrent enrollment are granted most commonly for courses and programs that ABU is unable to offer. 

  • Graduation Application

    The application must be completed after the student has registered for their final semester at ABU.

  • FERPA Consent Form

    Students must submit the FERPA Consent Form before ABU will release the student's educational records to a third-party.

  • FERPA Grievance Form

    Students may file a complaint if they believe their rights have been violated under the Family Educational Rights and Privacy Act. Email the completed complaint form to FERPA.Complaints@ed.gov

  • Verification of Enrollment Request

    Request a letter to verify the student's enrollment or un-enrollment from ABU.

  • Campus Housing Waiver Application

    Students who are not living at home with parents, grandparents, or a legal guardian MAY REQUEST permission to live off campus.
